ATTENDANCE: I understand that I need to give notice via phone, text, or email if my child will be absent or tardy from after school hula/activities. My child and I understand that attendance is taken daily, and my child is to check-in at Room 22 no later than 2:30p (Mo/Tu/Th/Fr) and 1:00p (We).

MAKE-UP CLASSES: I understand that if my child is absent, he/she can do a make-up any day later that semester.

NON-OPERATIONAL DAYS: I understand there will be no after school hula/activities on state and school holidays, other "no-student" days (per the PCHES calendar), half days, intersessions, and during any unforeseen school closures (for extreme weather, threats, facility emergencies like flooding, loss of electricity, etc.).

SCHOOL CLOSURES: In the event of an unforeseen or emergency school closure, I understand that I or my emergency contact will need to pick-up my child as soon as possible, and that I may be informed of a new safer pick-up location that my child may be transported to if the conditions at the school become unstable or unsafe in any way.

SUBSTITUTE: I understand that every effort will be made to arrange for a suitable substitute if the after school instructor(s) get injured or become ill. If a suitable substitute is unable to be found, for the best interest of my child, after school activities will have to be canceled (and a refund will be credited to me for the cancelation).

ILLNESSES: If my child is noticeably ill (presenting with a fever, Covid symptoms, etc.), I understand that I or my emergency contacts may be asked to take my child home early, for their well-being and that of their peers.

PICK-UP: I understand that I must pick up my child no later than 5:30p, and that I will give notice by phone or text if I will be late. If my child is not picked up by 6:30p, I give consent for my child to be driven to a new safer pick-up location (i.e. nearby open public library or shopping center) if the school lot becomes deserted or otherwise unsafe.

MONTHLY TUITION: I agree to pay the monthly tuition costs by the first class of each month. I understand that if my payment is not made by the 7th of the month, a $10 late fee may be assessed. I acknowledge that after the 15th of the month, my child may not be able to participate in hula/activities until delinquent payment + late fee is received.

DISENROLLMENT: I understand that I can disenroll my child at any time with a simple notification by email to pcafterschoolhula@gmail.com. I acknowledge that a prorated refund of the prepaid month can only be issued to me if I disenroll my child before the 15th of the month.
​
RULES: My child and I understand that it is my child's responsibility to follow all rules, as they are there for their safety as well as that of their peers, and that continued disregard for rules may result in dismissal.
